The short answer: Most recurring compliance paperwork fails for the same structural reason. The document that carries the standard and the document that captures the result are the same file, so every execution is a copy, every copy is editable, and the set can never be added up. A controlled master is designed once, by the person who owns the process, and then executed rather than copied. Three properties make it different from a template.
The standard is locked in place. Inside the document's embedded spreadsheet, the cells that hold the specification, the tolerance, the deliverable description, the acceptable quality level, are locked. The person executing the document cannot edit what they are being measured against. The cells that hold observations, measured values, dates, and dispositions are fillable. Every execution therefore measures the same thing.
Each execution produces a version-locked record. When a record is finalized, the content is frozen and stamped. The version reviewed is the version preserved, and later changes create a new version rather than overwriting the old one. That is what makes an individual record defensible on its own.
The aggregate is generated, not assembled. Because the results were captured as data in known cells, the set of records compiles into a scored summary on demand. Nobody opens the individual files to produce it.
The analogy that lands with most teams: a template is a blank form you photocopy, and a controlled master is a form printed on numbered carbonless paper that the office keeps a copy of. The second one produces a file cabinet you can actually audit.
The reason this is worth generalizing is that the subject matter changes completely and the mechanism does not. Below are two narrated walkthroughs, about a minute each, with captions. They share no vocabulary. They run the identical structure.

The deviation is visible because the tolerance was locked in the master and the measured value was captured as data, not as a sentence someone wrote in a comment field.
Strip the domain language out of both and you get the same four elements: a controlled document, a standard that must not move, a record per execution, and an aggregate somebody has to defend. Anywhere those four appear, the pattern applies. Inspections, audits, safety walkdowns, calibration, site surveys, periodic vendor reviews, and grant monitoring all have the same shape.

A controlled master does not make the judgment. It does not decide whether a deliverable was acceptable or whether a deviation is material. It removes the re-keying, the drift, and the manual rollup so that the person with the authority spends their time on the call rather than on the clerical work around it. It also does not retroactively fix records created the old way. Migrating a historical set means re-executing against the master, which is worth doing for an active contract or an in-flight qualification campaign and usually is not worth doing for a closed one.
